HOME HEALTH AGENCY
Colusa
Regional Medical Center’s Home Health Agency has
proudly provided home health services to our
community since 1986. We are a California licensed,
Medicare Certified and Joint Commission accredited
agency. We offer multilingual staff and interpreter
services. Our agency is trusted by physicians,
patients, and families alike to provide quality home
health care to our community. Services are covered
by Medicare, Medi-Cal and most insurance plans.
HOME HEALTH
TEAM SERVICES AVAILABLE
Our team
of caring home health care professionals and
paraprofessionals provide home health services in
the privacy and comfort of your home. Services are
coordinated and supervised by a registered nurse 24
hours a day, 7 days a week.

AVAILABILITY OF SERVICES
Physicians, case managers, discharge planners,
medical providers, caregivers, family members and
patients are encouraged to contact our intake staff
at (530) 458-2075 for referrals and to request a
clinical home assessment.
SKILLED
NURSING
Home Health
provides
its patients with a
comprehensive assessment of health and social needs and,
with the physician's order, establishes a plan of care
to meet those needs. We promote and
restore health and independence through the
provision of direct skilled nursing care and
individualized health education for disease
management and self-care. We provide and
teach high-tech infusion therapy, wound care and specialized medical treatments and procedures as
ordered by the physician.
MEDICAL
SOCIAL WORKER
CRMC's
Licensed Clinical Social Worker assesses
the patient for social, emotional and financial problems and
provides coordination of community resources and
long-term care planning.
PHYSICAL
THERAPY
Our
Physical Therapy staff evaluate safety of the home environment,
the patient's physical strength and
mobility, and develops a rehabilitation plan to restore
function and teach patients and caregivers physical
exercises and activities to achieve maximum
independence at home. We recommend medical equipment
and home modifications to promote safety and
independence at home.
OCCUPATIONAL THERAPY
CRMC's
Occupational Therapy staff evaluate the patient's ability to perform activities of daily living such as dressing, bathing, home maintenance, and
meal preparation. We Initiate a rehabilitation plan
to restore activities of daily living, and train
patients and caregivers in the use of assistive
devices to promote independence.
SPEECH THERAPY
Our Speech Therapy staff evaluate the patient's speech and swallowing abilities and
establishes a therapy regime for restoration of
cognitive functioning, swallowing deficits and
communication skills.
CERTIFIED HOME HEALTH AIDE
When other skilled services are required and, when
necessary, provides short-term personal care for
bathing, dressing, skin care, toileting, mobility
and other activities of daily living under the
guidance of the registered nurse or therapist.
SPECIALIZED HOME HEALTH PROGRAMS
In
addition to a wide array of home health services,
CRMC's Agency has worked to craft the following specialized programs to meet the
unique needs of our patients:
Maternal Newborn–
skilled
nursing care is tailored to maintain the health of new
mothers and newborns after delivery.
Palliative Home Care–
a
comprehensive, multidisciplinary palliative care
program allows supportive medical, social,
emotional and spiritual services to those
approaching end of life or a chronic life
threatening illness.
